For a faster consultation, please complete the questionnaire below, and show the form to the receptionist at the clinic or you can go directly to the clinic and the receptionist will provide you with the questionnaire.

Explanation/Inquiry: The physician will explain the diagnosis thoroughly, as well as the treatment plan.
Counseling: If you have further concerns, we can provide you with counseling.
npatient Referral: If we can’t deal with your medical concerns in our clinic due to lack of facilities, we will refer you to other specialist with the appropriate facilities.

After the examination, the receptionist will call out your name for the bill. |

After which, the doctor’s prescription will be given by the receptionist. |
